Surviving the Fourth Trimester: Why Chiropractic is a Postpartum Game Changer

What are the benefits of postpartum chiropractic care?

For nine months you focused on making space for your growing baby, maintaining your spinal alignment, and keeping your nervous system adaptable as your body built a whole freaking baby!

From the moment you deliver, your body enters an entirely new season, both mentally and physically.

Many of our moms assume that chiropractic care takes a back seat during these initial weeks of postpartum life. They believe they need to wait until their six-week appointment to startup care again. But waiting those long six weeks means leaving a highly stressed, rapidly changing nervous system to adapt to extreme stress entirely on its own.

At our specialized family practice here in Bismarck, we love seeing our postpartum moms get their nervous systems checked as soon as they feel ready! Let’s chat about when this care is available to you, what the process looks like, and why it can be a foundational pillar for your fourth-trimester recovery.

When can I get my first postpartum adjustment?

If you have experienced a birth with major complications or an extended hospital stay, we’d love to see your return to the office within the first 1 to 2 weeks postpartum.

You do not need to wait for your six-week checkup to get adjusted. The six-week clearance is designed to make sure that your uterine lining has healed, your cervix has closed, and any structural tissues (like a C-section incision or perineal tears) are sufficiently healed to clear you for exercise or intimacy. An OB/GYN is not licensed to give advice on when it is or isn’t ok to get adjusted again.

The good news for you is that chiropractic care during the first few weeks focuses directly on your body’s master control system, your brain and spinal cord. The sooner we remove the neurological stress of birth from your system, the faster your body can transition out of survival mode and into an active state of repair.

What happens during the first visit?

If you are imagining a deep, dynamic adjustment similar to one you may have received before pregnancy, go ahead and take that off your list of worries. Your early postpartum body requires an entirely different approach. Your ligaments are still flooded with the softening hormone relaxin, and your neuromuscular system is highly sensitive.

The first postpartum visit is meant to be a gentle, intentional, and restorative experience for your newly postpartum body.

Our Chiropractic Assessment

We begin with a conversation about your birth experience. If this is the first time you’re coming to see us, there can be a lot to catch up on! We love to understand how your pregnancy and birth went to build the best picture of how stressed your nervous system might be. Understanding the specific stressors of your labor, whether it was an intense, rapid delivery, a prolonged multi-day event, an emergency intervention, a scheduled C-section, or even a beautiful, uncomplicated birth, helps us identify how your brain has processed this experience. This helps us map exactly where trauma or stress has localized within your nervous system.

Our Adjustment

Because your postpartum body is still highly mobile and has freshly stressed tissues, we avoid aggressive manual adjustments and twisting you like a pretzel in side-posture. Instead, we use gentle techniques designed to complement where your body is in its healing journey. We may use light tonal adjustments, gentle drops, or even instrument-assisted adjustments with an Activator during your first couple of adjustments.

We typically prioritize the two primary areas of your central nervous system:

  • The Upper Cervical Spine: Our focus is on providing immediate relief from the strain caused by nursing, rocking, and constantly looking down at your newborn.

  • The Sacrum and Pelvis: Aimed at carefully restoring alignment to your body’s foundation while protecting loose, healing ligaments.

Why the First Postpartum Adjustment Is So Important

The first week at home can be a whirlwind as you adapt to your new, exciting life! Your body is navigating the steepest hormonal crash you’ll ever experience, shedding excess blood volume, and operating on next to no sleep. So basically, you’re living your best life!

Your first postpartum adjustment serves three critical areas during this time:

Managing Postpartum Anxiety

The intensity of birth, followed by the high alert of caring for a newborn, can keep your nervous system stuck in a state of sympathetic dominance. This is the part of your nervous system that controls your “fight, flight, or freeze” response. When your cortisol (the body’s primary stress hormone) remains elevated, it can slow tissue repair, disrupt sleep, and increase your risk of postpartum anxiety.  Providing gentle adjustments to your sacrum and upper cervical spine can help stimulate your vagus nerve (your body’s biggest parasympathetic nerve). This signals the brainstem to transition from survival mode into a “rest, digest, and restore” state, which is necessary for healing, mood regulation, and rest.

Supporting Pelvic Floor And Core Recovery

After birth, your body’s perception of its new center of gravity will drastically shift. During this time, if your pelvic bones or sacrum are left misaligned, your brain creates a map of its new posture around a twisted foundation. Early adjustments clear the nerve interference traveling from your pelvis to your brain, allowing your nervous system to accurately re-map your core alignment, stabilize your lower back, and create a safe mechanical environment for your pelvic floor to heal.

Can Chiropractic Care Help Breastfeeding and Milk Supply?

If you are choosing to breastfeed, week one is when your mature milk supply comes in. This can often bring feelings of engorgement, inflammation, and extra physical stress as you adapt to yet another change in your postpartum body. The pathways in your brain that control your milk let-down reflex and the blood flow to the mammary glands exit your spinal cord directly through your upper mid-back (thoracic segments T1–T4).

After spending hours curled over your baby in that lovely "nursing posture,” tension and stiffness in this region can start to become unbearable. A gentle adjustment in your early postpartum and beyond can help ensure that the neurological lines of communication are clear, supporting a comfortable, regulated milk supply and helping prevent early lactation bottlenecks.

Why your postpartum recovery plan is so important

Motherhood demands an extraordinary amount of physical and emotional resilience. All too often, your recovery is sidelined the moment the baby arrives. But a healthy, regulated baby requires a healthy, regulated maternal nervous system.

Bringing your newborn into our office during those first two weeks isn't just about getting your baby checked; it’s about ensuring your body has the proper safety nets it needs to navigate the fourth trimester with the rockstar energy we both know you have, even if it’s buried deep deep down under sleep deprivation.

Whether you’ve recently delivered or are preparing your postpartum plan, let’s ensure your recovery starts on a solid foundation. Contact our office to schedule your postpartum evaluation and get yourself on the right track.
